District‑wise distribution shows Hassan leading with **1,46,025** forms, followed by Davangere (1,07,802), Kalaburagi (79,375), Yadgir (78,280), Belagavi (75,052) and Tumakuru (66,304). In the Greater Bengaluru Authority (GBA), the North division distributed **23,221** forms, Central **12,122**, and South **10,620**, totaling **60,368** forms for the GBA.
