Historical Context

Launched on October 29, 2024 (9th National Ayurveda Day) to provide a dignity-based healthcare model for the elderly.

Exclusion Criteria

  • Seniors below the age of 70 (they are covered under standard PM-JAY if eligible).
  • Those who choose to continue with their existing government insurance (CGHS/ECHS) cannot dual-avail.

Sub-Schemes

PM-JAY Family Top-up

Shared family top-up for seniors in already covered families.

Vaya Vandana Individual Cover

Dedicated individual cover for seniors in families not covered by PM-JAY.

Challenges

  • Inadequate geriatric care specialists in small towns.
  • Digital literacy barriers for seniors to register for the Vaya Vandana card.
  • Ensuring hospitals do not 'cherry-pick' or deny treatment to the very elderly.
  • Potential for increased insurance premiums for the government due to higher risk in the 70+ age group.

Reforms & Recommendations

  • Mandatory creation of geriatric care units in all empanelled hospitals.
  • Inclusion of home-based palliative care under the scheme.
  • Linking with 'Senior Citizen Welfare Fund' for medicines.

Critical Analysis

This is a landmark expansion of India's social security net, recognizing the 'silver tsunami' (aging population). By removing income barriers for those 70+, the scheme avoids the 'exclusion errors' common in BPL-based targeting. A critical feature is the top-up for existing PM-JAY families and the option for those on private/other public insurance to switch. However, the fixed 5-lakh cover may be insufficient for complex geriatric surgeries, and the lack of dedicated geriatric wards in empanelled hospitals remains a supply-side bottleneck.
